The High School Transition project is funded by a grant from the Pennsylvania Developmental Disabilities Committee.   The project works with individuals who are currently involved in the transition process from high school to whatever they choose for themselves next.   The project uses person centered planning techniques to provide the individual and the family with realistic goals to help make the process as easy and organized as possible.
